Intake Form   Senior Adult ID Registry  

Our program is open to everyone regardless of age. Homebound residents of Santa Monica, Pacific Palisades, Malibu, or Topanga Canyon who are unable to shop or cook for themselves are eligible. Please feel free to call us and check if you have any questions or doubts.

To start we need to do an "intake." This is where we get all your delivery information and special needs. We will need the name, address and phone number of your Doctor so we can send him a "Diet Request Form" confirming your dietary needs. We will also need name and phone number for two people who will be your emergency contacts.

As we are funded by the cities of Santa Monica and Malibu, we are required to show that we are serving all age groups, income groups and all ethnic backgrounds. You have the option of filling out and submitting the "Intake Form" online or you can print the form and fax it to us at 310-857-7802. If you prefer, you can call our Client Coordinator between the hours of 9:00 AM and l:00 PM. Monday through Friday at 310-394-5133 and we will complete the intake over the phone.

Senior Adult Identification Registry

The Santa Monica Police Dept. offers a service to give peace of mind to our City's senior citizens, their families and caregivers by creating a database of ID and contact information.
